Companies that need LPMA to transport their food to the London Food Bank – Complete Contact Information and Section B – ONE FORM PER BUILDING. In order for us to facilitate prompt food pickup it is MANDATORY that a sign up sheet be completed for EACH building you are entering in the food drive. If possible, we ask that you combine pickups at a central location.

We will transport the food to the London Food Bank at 926 Leathorne St. London

We recommend that you display one poster on each floor of your buildings

SECTION B

We need LPMA to transport the food to the London Food Bank on April 28.

One form per building please. There is NO limit to the number of buildings you register, so please submit this form as many times as required. London only please.
